Nurses

The nursing program offers 3 types of services. 

Medication Management - In response to Medicare "Cutbacks",  Homefront is offering a medication management services.  The nurse will confirm all medications with the person's doctor.  A nurse will then visit weekly and set up the medication in a medication organizer.

Advocate services - Many people with health care problems, have trouble staying on top of information about their illness, medications, dietary restrictions, etc.  Our advocate service assists with management of all your health care needs.  At your request, we can arrange for the nurse to continue to visit on a weekly or monthly basis, a follow-up service that may prevent hospitalization and premature nursing home placement.
